Replacement Forms

The state provides forms to parent/instructors who have registered for a PTDE Program Guide with the TDLR (Texas Department of Licensing and Regulation).  The forms below are to be considered replacements and NOT originals.

Application For License
(DL-14A)

This form is the application for a learner/driver’s license. The DL-14 will be filled out with the student as the applicant and turned in at the local DPS office when applying for the learner license and again when applying for the driver’s license.  There are two different forms based on your age. If you are 17 years old and 1o months used the DL-14A. If you are under 17 years and 10 months, use the DL-14B.

Texas Residency Affidavit
(DL-5)

This form (DL-5) is used to confirm the student as a legal Texas resident. The DL-5 is to be used when applying for a learner license and will be turned in at the local DPS office.

*This form is filled out with the student being the applicant and the parent/guardian as the representative. The Parent/guardian will bring two supporting documents listed in the residency affidavit.

Verification of Enrollment
(Home School VOE)

This form (VOE; Verification of Enrollment) is used by students that homeschool. Students in public, private, charter, etc.. schools will request a VOE from the attendance clerk. The VOE will be required when applying for both the learner and driver licenses at the local DPS office.

*VOEs will usually expire after 30 days. However, during the summer months, they may extend to 90 days.

Road Rulz DL-91B
(Behind-The-Wheel Log Record)

This form (DL-91B) is the log record for the Behind-the-Wheel instruction, observation, and practice. It is recommended that you print the DL-91B once the student receives their learner’s license. During the behind the wheel portion they will need to record driving dates and times. This form is used at the local DPS office when applying for a driver’s license. The local DPS may copy the form, but the parent/instructor is to retain it for their records.

DE-964 Forms (Samples Only)

Driving Portion Only
This sample demonstrates how to properly fill out the DE-964 certificate a student receives that is enrolled in the Driving Portion Only course. This certificate is to be used when applying for a driver license and will be turned in at the local DPS office.

Driver License
This sample demonstrates how to properly fill out the second DE-964 certificate a student will receive. This certificate is to be used when applying for a driver license and will be turned in at the local DPS office.

Learner License
This sample demonstrates how to properly fill out the first DE-964 certificate a student will receive. This certificate is to be used when applying for a learner license (permit) and will be turned in at the local DPS office.
